This is either the coolest tattoo ever or some really excellent Photoshop work. I’m honestly not sure which.

Edit: Just discovered that this tattoo was created by Dan Hazelton of Milwaukee, Wisconsin. The design, called a “tear out,” was tattooed in 1996 in ten hours over the course of three session. (07/17/2010)
